Many people choose to have decorative tattoos in youth, but later regret this decision. Laser removal has become the treatment of choice. Unlike other tattoo removal treatments, laser surgery is completely non-invasive and the most likely to keep skin intact and looking healthy.

Cosmetic medical lasers vaporize ink fragments in the tattoo, causing them to fragment and the body to absorb them naturally. A quarter-sized tattoo can take as little as 3-5 minutes to treat, but a minimum of approximately 8-10 repeat treatments will probably be necessary. How Long a Tattoo Session Takes and Who Should Have Tattoo Removal?

A good candidate for tattoo removal is medium to fair skin, with dark but not dense ink (black/brown). The preferred locations are the arm, chest, buttocks, or any place with good circulation. Medium or fair skin patients react well because the light will be able to selectively target the ink instead of the dark skin. Some tattoos disappear in one treatment, and others requiring 12 or more treatments for complete removal. Taking care of the treated area is the most important factor in removing a tattoo without a scar; post laser treatment will determine the success rate.

Dr. El-Attar can not regulate the speed of the fading process. For best result, it has been proven that the treating doctor must act conservatively rather that aggressively, to allow the surrounding tissue to relax in the weeks following the treatment. Non-professional tattoos can usually be treated with 1-4 treatments and professional tattoos can usually be treated with 6-10 treatments, however only the treating physician can determine how many treatments are needed to successfully remove any tattoo.

Is it Painful?

Laser Vein Removal Treatment: spider, varicose , and sclerotherapy.

Varicose veins:

Healthy veins return blood to the heart and lungs so it can be re-oxygenated. A system of valves makes this happen by allowing the blood to flow in only one direction. When valves fail or leak, blood flows backward under the force of gravity and distends the veins. This backward flow, called “reflux,” causes blood to pool in the veins, resulting in varicose veins.

Ignoring the situation can result in symptoms such as pain, fatigue, itching, burning, swelling, cramping and throbbing. People who suffer from these symptoms instinctively lie down and elevate their legs to reduce the pooling of blood and relieve their discomfort. However, over time, these symptoms will intensify, and quite often, a person’s varicose veins will become large and “ropelike.” Overly distended varicose veins, especially near the ankle, can rupture and cause profuse hemorrhaging. Some legs can even deteriorate with swelling and dermatitis, which can lead to leg ulcers.

Spider veins:

Spider veins appear to be hormonally induced and can be associated with pregnancy and menstruation. Although spider veins are not usually symptomatic, the feeder veins deeper in the skin can cause discomfort. Many women have a combination of both varicose and spider veins. While they may seek treatment for cosmetic improvement, most female patients seek relief from pain.

Spider veins grow from “reticular” veins, also known as “feeder” veins. These feeders exist in a dense network, and are usually a bit deeper in the skin. They form a network of blue or green veins, sometimes giving a marbled appearance to the skin. They might even be difficult to see in some women because they are a little too deep in the skin. But they’re present nonetheless and must be eradicated in order to get the best results possible, while reducing the chance of recurrence. Many women, not knowing the proliferative nature of spider veins, don’t want them treated. These women figure as long as the veins aren’t too ugly, they can avoid treatment and save money. But in reality, when left untreated, spider veins provide a platform for the growth of additional spider veins.

Treatment options: Sclerotherapy versus Laser

We not only treat the unsightly surface spider veins, but also the feeders that actually “feed” them. Treating the feeders treats the source of the problem. This allows us to greatly improve our already sound, long-term results. This procedure is performed in our office and takes less than an hour.

Sclerotherapy is the process of eliminating varicose veins and telangiectasias (“spider-veins”) by injecting a solution (sclerosing agent) into the veins, and thereby dissolving
them.

Laser Vein Treatment: The laser beam passes through the skin and penetrates the targeted vessel. The laser energy heats up and coagulates blood inside the vessel. These vessels later dissolve within the body. You may experience a stinging sensation during the treatment, which resolves immediately after the treatment is over.

Is sclerotherapy for me?
A majority of people who receive sclerotherapy are cleared of their small varicosities, or
see a large improvement. Approximately 10% of patients have fair to poor results, meaning the veins have not disappeared after 6 treatments. Very rarely, the veins can become worse after treatment or new veins may develop. You are a candidate for sclerotherapy if you have spider veins, reticular varicosities, recurrence of varicosities, are advanced in age, or any combination of these.  During your consultation Dr. El-Attar will determine the best plan for you.

Contraindications for sclerotherapy: Pregnancy, patients who are bed-ridden or experience decreased mobility, diabetic patients, obesity, history of deep vein thrombosis, allergies to sclerosing agents, and hypercoagutable states (Lupus, etc.).

How many treatments are required?
The number of treatments is dependent upon a.) the extent of the varicosities and b. ) the patient’s agreement to follow post-treatment instructions. One to six treatments may be necessary, and the average is usually three to four.

What are the most common side effects?
Itching — This depends on the type of solution used. You may feel itching along the route of the vein, normally lasting 1-2 days after treatment.
Telangiectatic making (hyperpigmentation) — This occurs in approximately 10-30% of patients. You may notice a slight discoloration of brown streaks along the veins after treatment. Rarely, the darkening of the vein may last 4-12 months. It will usually not keep you from continuing treatment. Spontaneous resolution of the hyperpigmentation occurs in 70% of patients at 6 months, and in 99% of patients within 1 year.
Sloughing and ulceration — This type of reaction may occur at the injection site. It looks like a small blister and heals slowly. If the blister opens, or if it is not well taken care of, it may turn into an ulceration. A scar may develop, but it should return to normal color when the healing process is completed.
Allergic reactions — These reactions are extremely rare, but may occur if you are allergic to the sclerosing agent used. You may also experience an allergic reaction to the material in the compression stockings (Spandex and Lycra), however this is also rare.
Pain — You may experience mild to moderate pain at the injection site, with some possible bruising. The veins may be tender after the treatment, and an uncomfortable sensation may run along the route of the vein. This is temporary, usually lasting days.

What are the other side effects?
You may feel burning during the injection of the solution. Sometimes new tiny blood vessels develop after treatment, but this is usually temporary. Transient phlebitis-type reactions (swelling of the veins that may cause your ankles to swell), temporary wheels or blebs (similar to hives), and wound infection, poor healing or scarring may also occur. If an arteriole is injected it may lead to arterial embolism and/or pedal gangrene.

Phlebitis is a rare complication, occurring in approximately 1 out of every 1000 patients. If usually happens in the treatment of veins 3-4mm in diameter or greater. The dangers of phlebitis include: possible pulmonary embolus (blood clot to the lungs that could result in death), and post-phlebitis syndrome (when the blood is not carried out of the legs). This may result in permanent swelling of the legs.

Rosacea
This very common skin condition effects 5% of individuals. It causes ‘rosy cheeks’ and red patches or small blood vessels on the face. Patients often complain of an embarrassing ’sunburnt’ look. Rosacea is made worse by sun, heat/cold, and exercise; the face gets red, and stays red. Fortunately, there is medical laser treatment for this condition, but it is very important to treat it early. The doctor will discuss the laser treatment of Rosacea with you at your complimentary consultation, and assess whether you are a candidate for therapy.

Unwanted hair – nobody likes it, and nobody wants to have to deal with it. With traditional surgeries and techniques, hair removal has be painful, expensive, time-consuming, and worst of all, temporary.

With laser hair removal, say goodbye to the trouble, pain, and expense of getting rid of unsightly hair. Equally as effective for men and women, it works anywhere on the body, and has been proven to be permanent, efficient, and extremely affordable. Hair is removed from within the follicle itself, leaving a slim chance for regrowth. Depending upon the area being treated, the entire procedure can be performed in 15-30 minutes – a smaller amount of time than a lunch break.

The growth of human hair is cyclic, involving phases of active growth (anagen) and resting stages (telogen). In addition, an intermediate stage of transition and controlled regression, known as catagen, occurs between active growth and cessation of growth.

DURING THE ANAGEN PHASE: Hair-producing cells multiply and proliferate, producing more cells. The cortex cells in the middle of the hair follicle squeeze together into layers, and then harden. As this hardening process continues, the cortex cells cease to function, and eventually die. They are soon replaced by new cells.

DURING THE CATAGEN PHASE: The catagen phase is a relatively short growth phase of approximately 1-3 weeks in which the hair fibers and root sheaths cease growth. This leads to telogen, a ‘resting’ phase for the hair follicle.

DURING THE TELOGEN PHASE: The follicles stop producing hair and deposit their remaining cells on the end of the hair fiber, forming the anchor of the new, healthy fiber.

Q: What areas can be treated with laser hair removal?
A: Nearly any body hair can be treated. Dr. El-Attar is currently accepting patients in need of hair removal on the following locations: face, chest, back, underarms, arms, forearms, hands, bikini area, legs, and feet.

Q: How does laser hair removal work?
A: The laser works by a principle called Selective Photothermolysis. The laser is effective on the hair follicles in the active growing phase (Anagen). The laser is pulsed for a very short duration, (usually a few milliseconds) emitting a gentle beam of light on to the skin. The light energy passes harmlessly through the skin, but is absorbed by the pigment in the hair follicle and hair shaft. By absorbing this laser energy, the hair follicles are disabled, impairing their ability to grow.

Q: How does laser hair removal differ from electrolysis? Is the treatment permanent?
A: The main difference between this technology and the traditional method known as electrolysis is that lasers can treat multiple hair follicles at one time. It is no longer required to identify and probe each individual hair follicle, as is done with electrolysis. Laser hair removal is a relatively new technology. The FDA has only recently provided clearance that indicates lasers can achieve a permanent reduction in hair growth based on clinical tests with patients who have hair reduction two years after treatment completion.

As far as permanency goes, most people need an average of 5-7 treatments. Hair grows in cycles, and subsequent treatments are usually necessary to treat the follicles that are lying dormant below the surface of the skin. These dormant follicles contain little pigment, and therefore are often not affected by the first treatment.

Q: What laser is used for this procedure?
A: We use the Cynosure SmartEpil II TM Long Pulse Nd: YAG laser. The FDA has approved this laser system for permanent hair reduction for all skin types. This laser has a 1064nm wavelength light that specifically targets the pigment contained in the hair follicle and shaft. To diminish patient discomfort, the Zimmer cooling device uses cooling jets of air to minimize heat exposure to the epidermis and into the surrounding tissue before, during, and after exposure.

Q: What does the treatment include?
A: Depending on the amount of hair and area treated, the procedure varies from patient to patient. Safety eyewear is always used in order to protect the patient’s eyes from the laser light during the procedure. Most experience little discomfort during the treatment. The sensation and the degree of discomfort varies with each person. In some cases, topical anesthesia is an option. Dr. El-Attar will discuss this with you before your treatment begins. Patients can return to work and resume their normal daily activities immediately following treatment. The area may become slightly red for anywhere from a few minutes to an hour depending on the area(s) being treated and your individual skin sensitivity.

Q: What are the risks or possible side effects involved?
A: With the right machine and sufficient experience, you will not experience any damage to your skin. The only possible side effects are short term reactions such as a redness and follicular edema (which is like goose bumps), which may take from a few minutes to several hours to fade away. Most people can get treated during their lunch time and there will be no visible indications when they return to their office.

Q: Is it safe?
A: Absolutely. Laser hair removal has been being researched and developed for years. The laser parameters were carefully defined to inhibit regrowth by studying the anatomy of the hair follicle and matching the laser light and pulse duration to the follicle size, depth, and location to safely and effectively remove hair.

Q: What should I expect?
A: Around 3-10 days after treatment, new hairs will begin to surface. Most of these hairs were treated and are in the process of falling out. Rest assured that this is not regrowth. The underlying follicles need to purge the hairs that remained under the skin. Please refrain from waxing, plucking, or bleaching between treatments. Shaving is still acceptable and safe.

Congenitally prominent ears are a cosmetic deformity that can have a severe emotional and behavioral effect on a child. Otoplasty is a description of surgical procedures designed to give the auricle a more natural and anatomic appearance.

Problem: Prominent ears typically do not affect a child’s self-image until the child is older than 5 or 6 years, and surgery for lop ear deformity is best performed before this age. Conversely, adult patients often seek otoplasty to correct a long-standing defect. Before surgery, patients typically style their hair to camouflage their deformity. Postoperatively, patients often feel comfortable enough to wear their hair short or combed back.

Frequency: Five percent of white individuals are affected. Protruding ears are genetic; 59% of affected individuals have a family history, and transmission is in an autosomal dominant pattern with variable penetrance. Since the inner ear develops independently from the auricle, patients with prominent ears generally have normal hearing; however, other congenital deformities (especially of the genitourinary tract) may be present.

Clinical: Prominent ears can be in a myriad of forms, including cup ear, shell ear, bat ear, Machiavellian ear, and lop ear. Many patients attempt to camouflage the deformity with hairstyling.

Otoplasty is indicated for correction of ears that protrude more than 20 mm and at an angle greater than 35° from the occipital scalp. One or multiple malformed auricular subunits may be managed.

Medical therapy: Nonmedical management of lop ear deformity is generally achieved by the patient using effective hairstyling to camouflage the defect. Nonsurgical treatment of prominent ears is limited to the Far Eastern practice of applying tape or conforming bandages in early infancy to influence the growth and shape of the ear. Sustained restriction and pressure can guide the growth of the auricular cartilage during infancy. This is particularly useful for treatment of cryptotia, a condition in which the superior pole lacks lateral projection and is often covered by a fold of skin.

Surgical therapy: Otoplasty can be categorized as either cartilage splitting (cutting) or cartilage sparing. Cartilage-splitting techniques involve incisions through the cartilage and repositioning of large blocks of auricular cartilage. Cartilage-sparing techniques avoid full-thickness incisions, attempting to create more effective angles and curls in the cartilage.

Contraindications: Otoplasty is contraindicated in any patient with unrealistic expectations. Patients must receive appropriate preoperative counseling. Discuss existing facial asymmetries, and emphasize that a restoration of anatomic balance to the face is the goal of any surgery. Patients unable or unwilling to cooperate with postoperative care are not candidates for surgery. Advise patients with a history of hypertrophic scarring or keloids that these may occur after otoplasty, possibly distorting an otherwise excellent surgical result

Preoperative details: Generally, prominent ears are treated surgically. Proper preoperative management includes a detailed medical history, a psychological assessment (formal or informal), a careful physical examination, and photography.

Postoperative: The patient is seen on the first postoperative day. The dressings are removed, and the ears are inspected for any sign of hematoma. The drains are removed, and the ears are redressed, including conforming and compressive dressings. The patient is seen again on the second postoperative day; the ears are again inspected, and an elastic ski headband is placed over the head and ears. The patient wears this continuously for the next 5 days and while sleeping for the following 2 weeks. Prolene sutures are removed on the seventh postoperative day. Moderate ecchymosis and edema are expected but usually resolve within the first 2 weeks.

Incomplete correction of prominent ears is probably the most common undesirable outcome of otoplasty. Careful preoperative analysis with specific attention to each area of the auricle can prevent an incomplete reconstruction. Correction of the mid portion of the auricle greater than the superior and inferior poles leads to a “telephone ear” deformity; a reverse telephone ear deformity is a result of inadequate medialization of the central portion of the auricle.

Most patients can appreciate the results of surgery as soon as the bandages are removed. Some adult patients require a period of psychological adjustment to their new appearance. If patients are appropriately selected, they resume their lives with an improved level of self-confidence.

This procedure is also known by a variety of different names, such as the short scar facelift, baby boomer facelift, limited-incision facelift, or mini-lift. The S-Lift improves the visible signs of aging including jowls, neck sagging, and deep Marionette lines.

The best candidates for S-Lift

The S-Lift is valuable on patients in their forties and fifties with moderate amount of excess skin around their jaw line and neck. It is generally appropriate for patients who show sagging in the mid and lower face.

Planning your S-Lift

S-Lift is an individualized procedure. Prior to your surgery Dr. El-Attar will evaluate your face, including the skin and underlying structures, and discuss your goals for the surgery. Medical conditions that could cause problems during or after surgery, such as uncontrolled high blood pressure, blood clotting problems, or the tendency to form excessive scars will be ruled out. Be sure to inform your surgeon if you smoke or are taking any drugs or medications, especially aspirin or other drugs that affect clotting.If you decide to have the S-Lift, Dr. El-Attar will explain the techniques and anesthesia he will use, and the risks and costs involved. Don’t hesitate to ask any questions you may have, especially those regarding your expectations and concerns about the outcome. After the consultation you will sign an informed consent to have the procedure.

Preparing for your S-Lift

Dr. El-Attar will give you specific instructions on how to prepare for surgery, including guidelines on eating and drinking, smoking, and taking or avoiding certain vitamins and medications. Carefully following these instructions will help your surgery go more smoothly. If you smoke, it’s especially important to stop at least a week or two before and after surgery; smoking inhibits blood flow to the skin, and can interfere with the healing of your incision areas. If your hair is very short, you might want to let it grow out before surgery, so that it’s long enough to hide the scars while they heal. You should arrange for someone to drive you home after your surgery, and to help you out for a day or two if needed.

Where your S-Lift will be performed

The S-Lift is performed in our office-based facility in Mercerville, New Jersey.

Types of anesthesia for S-Lift

S-Lift is performed under local anesthesia. It can be combined with an oral sedative to make you drowsy. You’ll be awake but relaxed, and your face will be insensitive to pain. (However, you may feel some tugging or occasional discomfort.)

The S-Lift procedure

The procedure for an S-Lift includes repositioning all of the portions of the face that show signs of aging. The incision is approximately one-half the length of a traditional facelift incision. The incision extends from your temporal hair line to behind the ear. The surgeon separates the skin from the fat and muscle below. Fat may be trimmed from around the neck and chin to improve the contour. The surgeon then tightens the underlying muscle and membrane, pulls the skin back, and removes the excess. Stitches secure the layers of tissue and close the incisions.Following surgery, a small, thin tube may be temporarily placed under the skin behind your ear to drain any blood that might collect there. The surgeon may also wrap your head loosely in bandages to minimize bruising and swelling.

Complications

Complications are infrequent and usually minor. Still, individuals vary greatly in their anatomy, their physical reactions, and their healing abilities. The outcome is never completely predictable.Complications that can occur include infection, hematoma (a collection of blood under the skin that must be removed by the surgeon), injury to the nerves that control facial muscles (usually temporary), and reactions to the anesthesia. Poor healing of the skin is most likely to affect smokers.You can reduce your risks by closely following your surgeon’s advice both before and after surgery

To prevent complications it is important to:
[ ] Keep your head elevated as much as possible to reduce swelling
[ ] Apply ice packs to minimize swelling
[ ] Do not take aspirin or anti-inflammatory medications (your doctor can provide you with a list of OTC medications to avoid)
[ ] Do not smoke or be exposed to secondary smoke during recovery
[ ] Avoid exposure to direct sunlight
[ ] Use a sunblock (minimum 15 SPF) to protect your skin

After your S-Lift

There isn’t usually significant discomfort after surgery; if there is, it can be lessened with over-the counter pain medication. (Severe or persistent pain or a sudden swelling of your face should be reported to your surgeon immediately.) Some numbness of the skin is quite normal; it will disappear in a few weeks or months.Your doctor may tell you to keep your head elevated and as still as possible for a couple of days after surgery, to keep the swelling down.If you’ve had a drainage tube inserted, it will be removed one or two days after surgery. Bandages, when used, are usually removed after one to five days. Don’t be surprised at the pale, bruised, and puffy face you see. Just keep in mind that in a few weeks you’ll be looking normal. Most of your stitches in the face will be removed after about five days. Your scalp may take longer to heal, and the stitches or metal clips in your hairline could be left in a few days longer.

Recovery after your S-Lift

You should be up and about in a day or two, but plan on taking it easy for the first week after surgery. Be especially gentle with your face and hair, since your skin will be both tender and numb, and may not respond normally at first.Avoid strenuous activity, including sexual activity and heavy housework, for at least two weeks (walking and mild stretching are fine); avoid alcohol, steam baths, and saunas for several months. Above all, get plenty of rest and allow your body to spend its energy on healing.At the beginning your features may be distorted from the swelling, your facial movements may be slightly stiff and you’ll probably be self-conscious about your scars. Some bruising may persist for two or three weeks, and you may tire easily. It’s not surprising that some patients are disappointed and depressed at first. By the third week, you’ll look and feel much better. Most patients are back at work about ten days after surgery. If you need it, special camouflage makeup can mask most bruising that remains.

Pain Level: Moderate discomfort. 1-3 days of pain medication
Swelling and bruising: 2 weeks or more
Numbness: lasts several weeks
Bandages: Changed in 1-2 days (drains and sutures removed first 2-5 days)
Work: Return after 1-3 weeks
Exercise: Wait 2-3 weeks
Sun protection: Six months with SPF 15 or higher
Final result: Seen 1-4 months following most facelifts

Your new look after your S-Lift

Most patients are very happy with their S-Lift. Especially if you realize that the results may not be immediately apparent. Even after the swelling and bruises are gone, your hair may be thin and your skin may feel dry and rough for several months. You’ll have some scars from your S-Lift, but they’re usually hidden by your hair or in the natural creases of your face and ears. In any case, they’ll fade within time and should be scarcely visible. Remember, having S-Lift doesn’t stop the clock. Your face will continue to age with time, and you may want to repeat the procedure one or more times-perhaps five or ten years down the line. But in another sense, the effects of even one S-Lift are lasting; years later, you’ll continue to look better than if you’d never had another facelift at all.

Plan to spend a night or two before returning home. We will need to see you on the seventh day after your surgery for suture removal and wound check.

What is Microdermabrasion?

Microdermabrasion is the latest scientific method using fine inert (micro} crystals to progressively abrade the skin’ surface. It is used for skin resurfacing and treating a range of skin abnormalities. It can be described as “closed dermabrasion” where the dead cells, used crystals and any debris on the skin’s surface are collected in a waste bottle.

Who can benefit?

What areas can be treated?

Most common areas are the face, neck, chest, shoulders, back, hands, knees and elbows. Any body area can be treated.

Why microdermabrasion?

Approximately every 28 days we create a new layer of skin. As new cells form they push their way to the surface and then dead cells settle on the surface. This can cause enlarged pores, clogged pores, a bumpy uneven texture, and a dull appearance. As we age this process slows down leading to the appearance of older skin with less radiance.

How does it work?

The gentle exfoliation is performed with a jet of fine mineral crystals passing over the skin’s surface by creating a gentle vacuum. The crystals lay on the skin along with the dead and damaged skin cells. The crystals gently exfoliate approximately 20-25 microns of dead skin cells of the outermost layer (stratum corneum). Once removed it reveals one’s own fresher, younger looking skin beneath. One microdermabrasion is equal to three 30% glycolic peels or one 70% peel.

How many treatments do you need?

This treatment is meant to be progressive not aggressive. Generally results are more noticeable after the third treatment. Although, after the first session the skin will appear fresher, cleaner, smoother and softer for some clients. Usually treatments are performed every 7-10 days or every 2 weeks for a series of 6. Maintenance treatments are suggested once every one to one and a half months.

How long does it take?

The actual treatment usually takes 15-40 minutes, depending on the body area. The treatment is applied to clean skin. A moisturizing sun block is usually applied at the end of the treatment.

What are the side effects?

Since there is minimal discomfort and no healing time, one can resume normal activities immediately after. One may experience a slight tingling, and afterward the skin may feel tighter, as if exposed to the wind or sun. It may appear slightly flushed and this generally fades in 12-24 hours. It is very important NOT to EXERCISE for 24 hours after a treatment. Perspiration will produce a stinging sensation on newly treated skin which can take several hours to dissipate.

What other techniques can be used to make microdermabrasion even more effective?

Microdermabrasion can be combined with a new treatment called IPL (Intense Pulsed Light) – also known as Fotofacials. IPL treats broken blood vessels, background red skin tones, brown freckling, shrinks pores, improves sun damage and increases collagen in the skin. All of these lead to smoother, younger looking skin and reduced redness at the same time.
